Confirmation Requirements

Requirements are as follows:

• Families should be active parishioners at St. Patrick's Church (using numbered church envelopes or giving tithe electronically).
• All absences must be pre-approved and make up work must be submitted. More than 3 absences will require to make up all sessions missed. 
• All students must attend mass on Sundays and Holy Days of obligation and sign in and out using their QR code. 
• All students must be active in a church ministry to provide their community service to the church.  
• All students must live at least one (1) confirmation retreat at St. Patrick prior to receiving the sacrament.
• Parents and Sponsors must attend the adult portion of the retreat (4 hours.)
• 1st year & 2nd year parents are required to participate in at least 5 evangelization sessions provided for them every year at St. Patrick.

• Sponsor must be at least 16 years old having received the Sacraments of Baptism, Holy Communion, Confirmation and if applicable, married in the Catholic Church. 
• In order to serve as a role model and support the candidate, the sponsor must be a practicing Catholic, who fully participates in the sacramental life of the Church in living a full and active Catholic life in both word and deed. If a person does not attend Mass on a weekly basis, then he or she is ineligible to be a sponsor at this time.
• The sponsor must be someone other than the mother or father of the candidate.
• Only one sponsor (godparent) is required, either male or female.
• Please submit this form filled out by the sponsor and signed and stamped at the bottom by his/her parish, accompanied by the sacramental certificates Baptism, Confirmation and Matrimony.
• The deadline to turn is this form for 2nd Year students is August 4, 2022. No extensions.
